Five (5) Whys

Root cause Analysis – The 5 Whys  – The 5 why’s practice by Taiichi Ohno’s, is the method of asking why five times whenever a problem was encountered in order to identify the root cause of the problem, so that effective countermeasures could be developed and implemented. Used along with other problem-solving tools, five whys method enables you to derive the proper corrective action. This question asking method is used to explore the cause/effect relationships underlying a particular problem. Ultimately the goal of applying the 5 why’s method is to determine a root cause of a defect or problem.
